Kids Above All is seeking a talented Health Coordinator for our Emerge department.
Job Summary: The Health Coordinator position will utilize a variety of culturally appropriate outreach methods to provide health screening, referral, nutrition, and fitness information and services to low and moderate income families served by Kids Above All.
Essential Functions:
- Recruit clients both internally (among Kids Above All clientele) and externally
- Become familiar with health resources and services and become an active member of the public health network in the county
- Design and implement effective outreach activities at health and community fairs, schools, area churches, social service agencies, shopping malls and other public venues
- Conduct child and adult health screening
- Provide information about, and referrals to, area health resources and agencies
- Develop effective programming to communicate the benefits of healthy nutrition and active living for children and families
- Conduct family-centered activities and classes the encourage children to try healthy foods and to participate in fitness activities
- Link families to health resources that provide comprehensive primary care
- Empower families to pursue health care services through the provision of referrals and support
- Conduct follow-up assessments to determine the effectiveness of the program
- Prepare evaluation, outcome, and data reports as needed
- Participate in ongoing training and professional development
- Maintain job specific/core competencies
- Conduct home visits to clients.
Qualifications:
- Bachelor’s Degree in public health, health education or similar field
- Additional certifications and qualifications specific to nutrition, fitness and related fields are desirable
- At least two years experience providing public health services to a diverse client base
- Demonstrated knowledge of the design and implementation of effective community-based education programs focused on health, nutrition, and fitness
- Oral fluency in English and Spanish preferred.
- Ability to organize work and prepare required reports
- Must be flexible and willing to work evening and/or weekend hours, if needed, to carry out job responsibilities

Base Salary: $40,000
